The systematic collection of information from a patient as part of the individual’s medical history. Focuses on details related to gynecological and obstetrical health including but not limited to sexual practices, pregnancies, births, abortions, miscarriages, menses, partners, sexually transmitted infections, contraception, sexual activity, and past Pap test results.
